Securing the continuity of care for a patient with cancer by means of advance care planning

The purpose is to describe the evidence-based practices of advance care planning (ACP) in terms of data content, data structure and data quality from the perspective of professionals.
The aim is to ensure the continuity of care for a patient with cancer, to improve quality of care and patient safety by promoting the flow of information and preventing unnecessary emergency visits.
Preventing pressure ulcers in long-term care -development and evaluation of an intervention

The purpose of the study is to develop and implement an intervention based a renewed consistent practice for pressure ulcer prevention on international guidelines for pressure ulcer prevention, which focuses on older people in long-term care facilities and evaluate its effectiveness.
Sample and method
The sample of this study is of older people and staff in two long-term care facilities (five intervention wards and eight control wards: 223 patients, 141 nurses).
The data were collected with three structured questionnaires and from the patients’ records before, after (both in intervention and control wards) and during intervention (follow-up data only in intervention wards).
Development and implementation of pressure ulcer intervention in the four phases Operational Model for Evidence-Based Practices (OMEBP)
The four phases of development include:
1. Assess the development needs of PU prevention practice;
2. Plan for consistent practice in accordance with the evidence;
3. Describe and implement the renewed consistent PU prevention practice
4. Evaluate and follow-up of the renewed consistent PU prevention practice
The renewed consistent practice for pressure ulcer prevention was developed together during four development days with staff, researchers and experts in pressure ulcer. Before and during the implementation of intervention, staff members were trained in pressure ulcer prevention and documentation.
